Chronic Care Management is a specialized service offered to individuals who have a serious illness. You can continue to seek curative treatment while on CCM or continue to receive home health. The goal of CCM is to improve your quality of life. At Assisteo our CCM is provided by a specially trained team of doctors, nurses, nurse practitioners, and social workers.
